Job Description: Nurse Practitioner/Physician Assistant – House Calls
Position: Nurse Practitioner/Physician Assistant
Reports to: Medical Director
As a Nurse Practitioner (NP)/Physician Assistant making visits in patient homes, your roles and responsibilities will include:
- Patient Assessment: Conduct comprehensive health assessments, including physical examinations, reviewing medical histories, and identifying any changes in patients conditions.
- Diagnosis and Treatment: Utilize your advanced clinical knowledge and skills to diagnose medical conditions, develop treatment plans, prescribe medications, and order and interpret diagnostic tests.
- Medication Management: Monitor medication regimens, ensure adherence, and adjust dosages as needed. Educate patients and caregivers about medication usage, potential side effects, and drug interactions.
- Care Coordination: Collaborate with other healthcare professionals, such as physicians, nurses, therapists, and social workers, to coordinate and provide holistic care to patients. This includes arranging referrals, consultations, and follow-up appointments.
- Chronic Disease Management: Assist in managing chronic conditions by developing care plans, providing patient education, promoting self-management strategies, and monitoring disease progression.
- Health Promotion and Education: Promote preventive healthcare practices, educate patients and their families on healthy lifestyle choices, disease prevention, and self-care techniques.
- Documentation and Record Keeping: Maintain accurate and up-to-date patient records, including medical histories, assessments, diagnoses, treatments, and progress notes. Adhere to legal and ethical standards regarding patient confidentiality and privacy.
- Emergency Response: Be prepared to handle medical emergencies that may arise during visits and on non-visit time. Assess and stabilize patients, initiate appropriate interventions, and coordinate emergency medical services when necessary.
- Emotional Support: Provide emotional support and counseling to patients and their families, addressing their concerns, answering questions, and offering guidance throughout the healthcare journey.
- Quality Improvement: Participate in quality improvement initiatives, such as conducting audits, implementing evidence-based practices, and ensuring compliance with regulatory standards.
Terms:
- 1099/W2
- NP to Provide On-Call Medical Coverage to Patients on NP’s Panel
- NP/PA will be making house-call visits.
- Take a training course on standard operating procedures in MedBetterHealth University.
- Ensure all documentation is completed at time of visit.
Compensation to be as follows:
From $75-100 per visit.
The following will be the clinical responsibility of the Nurse Practitioner on his/her panel of patients on non-visit time:
- Reviewing labs
- Addressing critical labs and or critical imaging results
- Scheduling follow up appointments.
- Making oneself available should any urgent patient related matters arise.
Job Type: Part-time
Pay: $75.00 – $100.00 per hour
Expected hours: 8 per week
