POSITION SUMMARY
To provide superior quality, competitive value, and outstanding service by providing the highest standard of quality medical care to Brevard Health Alliance (BHA) patients through the primary care medical home model. Under the observation of their supervising Physician and under the Chief Medical Officer, provides extraordinary patient care throughout BHA Clinics.
GENERAL EDUCATION REQUIREMENTS
Master of Science in Nursing from an educational institution approved by the Florida Department of Health or Master of Science in Medicine, in Physician Assistant studies from an educational institution approved by the Florida Department of Health.
ADDITIONAL QUALIFICATIONS
- Clear and active Florida Physician Assistant licensure
- 3+ years’ experience of related clinical experience preferred. Experience in a primary health care environment, community health center or Federally Qualified Health Center (FQHC) preferred
- Experience working in various outpatient settings
- Current BLS/CPR certification from the American Heart Association or the American Red Cross
- Proficient in Electronic Medical Records (EMR)
- Demonstrates strong oral and written communication skills
- Extensive knowledge of physical assessment, differential diagnosis, pathophysiology
PRIMARY ACCOUNTABILITIES
- Manages acute, episodic outpatient care and health maintenance needs through the provision of a Primary Care Medical Home
- Demonstrates competent skills in therapeutic, cost-effective pharmacologic administration and management
- Develops and implements formal and informal teaching plans for patients, families, staff, and community
- Promotes patient/family-informed decision making in treatment plans and options
- Performs physicals, diagnostic tests, and screening evaluations
- Manages overall patient care regarding lifestyle and development issues
- Emphasizes preventative care and disease management
- Understands the changes in health promotion throughout the aging process, consults with the collaborating physician as warranted
- Collaborates with Chief Medical Officer in development of clinical protocols/procedures for delivery of outpatient care
- Provides care to patients in a safe environment
- Participates in departmental Continuous Quality Improvement efforts and provides support for improvement of identified issues
- Works with clinical manager to ensure collaborative problem solving with staff through participation in identification, mediation, and resolution of issues and team problems
- Promotes staff and community enhancement through coordination of in-services to staff, Physicians, and partners to promote profession and organization
- Attends monthly department staff meetings, Provider meetings, and other scheduled meetings
- Maintains Quality Department Performance measures
- Maintains predetermined productivity based on patient volumes
- Maintains internal customer satisfaction
- Takes ownership and represents Brevard Health Alliance’s mission and organizational goals established by the executive committee. These include, but are not limited to: Productivity, Quality, Patient Satisfaction, and Employee Retention. It is the duty of each BHA employee to incorporate these goals into all daily work routines and strive to achieve and surpass all goals by working as a team. The mission and goals of BHA will be incorporated into each associate’s Key Performance Indicators and will be evaluated on a monthly and quarterly basis.
