Nurse Practitioner Job Description

Job OverviewWe are seeking a compassionate and skilled Physician Assistant or Nurse Practitioner to join our healthcare team. The ideal candidate will have experience in various medical settings, including outpatient and hospital environments, and will demonstrate a commitment to providing high-quality patient care. This role involves working with patients of all ages, focusing on comprehensive assessments, treatment plans, and follow-up care, particularly in hospice palliative medicine and senior care.

Duties

  • Conduct thorough patient assessments, including health histories and physical examinations.
  • Develop and implement individualized treatment plans based on patient needs.
  • Manage patient care in both outpatient and hospital settings.
  • Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
  • Utilize electronic health records (EHR) systems, specifically Epic, for data management and documentation.
  • Collect and analyze data for continuous improvement of patient outcomes.
  • Provide education to patients and families regarding health management and treatment options.
  • Perform esthetic laser treatments as needed, ensuring patient safety and satisfaction.
  • Complete Minimum Data Set (MDS) assessments for appropriate patient classification.
  • Address aliment-related health issues through effective management strategies.
  • Assist with our rotating on call schedule to assist patients over the phone after hours

Skills

  • Strong knowledge of hospice palliative medicine principles.
  • Proficiency in data collection and management techniques.
  • Familiarity with Epic or similar electronic health record systems.
  • Experience in both outpatient and hospital settings is preferred.
  • Understanding of MDS processes for senior care evaluations.
  • Ability to perform esthetic laser treatments safely and effectively.
  • Excellent communication skills to interact with patients, families, and healthcare professionals.
  • Strong organizational skills to manage multiple patients and tasks efficiently.

Job Type: Full-time

Pay: From $115,000.00 per year
