We’re growing, and we’re looking for APPs who want to make a real difference for patients recovering in skilled nursing facilities.

In this role, you’ll be the consistent clinical presence for patients at your assigned facilities. You’ll work independently day to day, collaborate closely with ISG physicians, and have support from local and regional clinical leads. We invest in our APPs’ growth, and we reward the quality and volume of the care you provide.

What You’ll Do

• Complete admission H&Ps, follow-up visits, and discharge evaluations for patients in skilled nursing facilities
• Diagnose and manage acute and chronic conditions, including infectious disease, common in a post-acute population
• Order and interpret labs, imaging, and other diagnostics
• Prescribe and reconcile medications, with attention to polypharmacy and patient safety
• Work with facility nursing, therapy, and interdisciplinary teams on care plans and goals of care
• Help prevent avoidable hospital readmissions through early recognition and proactive management
• Communicate clearly with patients and families about diagnoses, treatment plans, and what to expect
• Maintain timely, accurate documentation that supports quality care and compliant billing

What You Bring

• Active Florida APRN license with national NP certification, or active Florida PA license with NCCPA certification
• Active DEA registration, or the ability to obtain one before starting
• Master’s degree from an accredited NP or PA program
• At least 1 year of NP or PA experience preferred; post-acute, geriatric, or internal medicine experience is a plus
• Working knowledge of post-acute care regulations and documentation standards
• Strong clinical judgment and the confidence to practice independently
• Clear, compassionate communication with patients, families, and care teams
• A collaborative, positive attitude and a commitment to continued learning

Schedule & Location

• Monday through Friday daytime schedule, with flexibility in how you structure your day at your facilities
• Participation in a night/weekend on-call rotation
• Onsite at skilled nursing facilities across Hillsborough and Pinellas Counties; this is not a remote position

Compensation & Benefits

• Base salary of $108,000–$125,000, depending on experience
