Full Job Description
Family Centers seeks a Physician Assistant (Primary Care) or Nurse Practitioner (FNP or PNP) to deliver comprehensive primary medical care to clients in a School-Based Health Center. The School Based Health Center provides continuous and comprehensive services necessary for the promotion of optimal health for students.
The Role:
The Primary Care Provider will be responsible for providing preventative and acute care for clients within the School Based Health Centers. This includes management of acute and chronic illnesses, referrals, vaccines, screenings with an emphasis on promoting optimal health and preventative care. The Primary Care Provider will collaborate with an interdisciplinary team of providers within the SBHC and the larger agency. This is a full-time, 10 month position.
Education and Experience:
- Master’s or post-Graduate’s degree from an accredited program
- Current active license to practice in the State of Connecticut
- Minimum of 2+ years related experience and/or training is preferred
- Demonstrated aptitude in providing medical services to under-served populations.
- Experience in treating the Pediatric population is preferred
- Excellent multitasker, leadership, and team oriented
- Bilingual (Spanish) is a plus.
- Medical malpractice insurance (in addition to that offered by the agency)
- Current active DEA license
- Infectious Control Certified a plus
