Full Job Description
Family Practice Physician Assistant or Nurse Practitioner needed for an outpatient role with a CT-based Community Health group just outside Middletown, Connecticut. Physician Assistant and Nurse Practitioner providers with prior Family Practice APP experience will be considered. This is a FQHC Opportunity – hiring as of 12/4/2024.
Family Practice Nurse Practitioner / Physician Assistant – Roles | Responsibilities:
- Provide Family Practice clinical services to patients at one site in the Middletown, Connecticut area.
- Coordinate with other clinical (including Physician Assistants / Nurse Practitioners) and non-clinical staff to provide optimal patient outcomes.
- Manage acute and chronic conditions for Family Practice patients.
- Prescribe medication for patients as required or directed.
- Coordinate with specialists as needed.
- Provide education to patients and family members when necessary.
- Medical Spanish is a plus.
Days | Hours | Compensation | Benefits:
- Full-time = 40hrs/week
- Part-time = 20hrs/week minimum
- Administrative / Non-Clinical time provided
- Some preferred schedules will be considered
- Very minimal call – no physical call
