Nurse Practitioner (APRN) or Physician Assistant (PA)Job Type: Full-TimeSalary Range: $100,300 – $161,320 per yearWork Environment: In-person

Position Summary:

Community Health Services (CHS) is seeking a compassionate and experienced Nurse Practitioner (APRN) or Physician Assistant (PA) to join our high-performing primary care team. This role is ideal for a dedicated clinician committed to delivering comprehensive, culturally competent care in a collaborative, community-based setting. As a Federally Qualified Health Center (FQHC), CHS is proud to serve a diverse patient population while fostering a professional environment that values clinical excellence, collaboration, and compassion.

Key Responsibilities:

  • Deliver high-quality, evidence-based clinical care in accordance with established standards and protocols.
  • Perform comprehensive assessments, obtain medical histories, diagnose conditions, and develop appropriate treatment plans.
  • Collaborate with a multidisciplinary care team to ensure continuity and coordination of patient care.
  • Document all patient interactions and clinical findings in a timely, accurate, and thorough manner using the electronic health record system.
  • Educate and counsel patients regarding medical conditions, treatment plans, preventive care, and health maintenance.
  • Participate in quality improvement initiatives and comply with organizational and departmental performance measures.
  • Provide culturally sensitive care to patients from diverse backgrounds.

Minimum Qualifications:

  • Graduate of an accredited school of nursing and an approved nurse practitioner program, or graduate of an accredited physician assistant program.
  • APRN or PA license in the State of Connecticut (required or must be eligible to obtain).
  • DEA and Connecticut state narcotic registration (or eligible to obtain).
  • 1–2 years of clinical experience in an ambulatory or urgent care setting is preferred.
  • Proven knowledge of advanced nursing or physician assistant clinical theory and practice.
  • Strong interpersonal and communication skills, both written and verbal.
  • Proficiency in electronic health records (EHR) and related computer systems.
  • Ability to deliver patient-centered care and contribute to a collaborative care environment.

Preferred Skills:

  • Bilingual, Spanish-speaking candidates preferred.
  • Demonstrated cultural competence in working with underserved populations.
  • Proven ability to make independent, evidence-based clinical decisions.
