Position Summary:
Community Health Services (CHS) is seeking a compassionate and experienced Nurse Practitioner (APRN) or Physician Assistant (PA) to join our high-performing primary care team. This role is ideal for a dedicated clinician committed to delivering comprehensive, culturally competent care in a collaborative, community-based setting. Key Responsibilities:
- Deliver high-quality, evidence-based clinical care in accordance with established standards and protocols.
- Perform comprehensive assessments, obtain medical histories, diagnose conditions, and develop appropriate treatment plans.
- Collaborate with a multidisciplinary care team to ensure continuity and coordination of patient care.
- Document all patient interactions and clinical findings in a timely, accurate, and thorough manner using the electronic health record system.
- Educate and counsel patients regarding medical conditions, treatment plans, preventive care, and health maintenance.
- Participate in quality improvement initiatives and comply with organizational and departmental performance measures.
- Provide culturally sensitive care to patients from diverse backgrounds.
Minimum Qualifications:
- Graduate of an accredited school of nursing and an approved nurse practitioner program, or graduate of an accredited physician assistant program.
- APRN or PA license in the State of Connecticut (required or must be eligible to obtain).
- DEA and Connecticut state narcotic registration (or eligible to obtain).
- 1–2 years of clinical experience in an ambulatory or urgent care setting is preferred.
- Proven knowledge of advanced nursing or physician assistant clinical theory and practice.
- Strong interpersonal and communication skills, both written and verbal.
- Proficiency in electronic health records (EHR) and related computer systems.
- Ability to deliver patient-centered care and contribute to a collaborative care environment.
Preferred Skills:
- Bilingual, Spanish-speaking candidates preferred.
- Demonstrated cultural competence in working with underserved populations.
- Proven ability to make independent, evidence-based clinical decisions.
