Job Role
- Provide On-Site or Telehealth Consultations: Conduct New Patient health history intake, complete thorough patient exams & assessments, and evaluate sleep patterns and symptoms. You’ll coordinate at-home sleep testing and implement personalized treatment plans that may include lifestyle changes, and appropriate medical interventions to guide them toward their health goals
- Offer Preventive Care: Help patients address sleep hygiene, stress management, and other lifestyle factors that impact sleep. Engage in proactive care to prevent sleep disorders from escalating.
- Monitor and Adjust Treatment Plans: Follow up with patients through telephone or video calls to track progress, adjust treatment strategies, and provide ongoing support to ensure optimal results. Review test results and work closely in a collaborative team environment with the team, clinic owner, medical director and collaborating providers/specialists regarding patient continuum of care.
- Stay Updated: Participate in continuous education and training to remain on the cutting edge of sleep medicine and telehealth. Maintain professional and technical knowledge by reviewing professional publications; establishing personal networks; and participating in professional societies.
- Document Care: Maintain accurate, up-to-date patient records in compliance with HIPAA, OSHA & regulatory standards. Provide concise, detailed progress notes after each consultation.
- Patient Education: Generate educational content in support of our online blogs and patient portal content library
- Referring Provider Relationships: Collaborate with referring providers and other care team members to ensure continuity of care. Foster strong relationships with local providers to support referral growth to include educational outreach and Lunch & Learns or Zoom Meetings.
Qualifications
- Licensed Physician (PA) or Nurse Practitioner (NP) in Colorado with current and unrestricted state licensure and Active BLS certification required.
- One – Two Years’ Experience as a PA or NP preferred. Specific sleep medicine experience not necessary as training will be provided.
- Empathetic and Patient-Centered: You are a compassionate provider who is skilled in listening to patients and offering actionable solutions. You will build long-term relationships with patients and support them throughout their treatment journey.
- Strong Communication Skills: You excel in communication and can explain complex medical concepts to patients in an easy-to-understand and empathetic way.
- Technology-Savvy: You are enthusiastic about integrating technology into sleep health care models. Familiar with using video calls, scheduling tools, and digital health platforms. Experience with computers including electronic medical records required. Microsoft Office (Word, Excel, Outlook), and Adobe PDF documents preferred.

- Independent Leader: You are self-motivated to seek & suggest opportunities for growth and the ability to work autonomously with appropriate on- or off-site supervision/collaboration from a medical director.

Pay: $80,000.00 – $130,000.00 per year
