Nurse Practitioner or Physician Assistant – Primary Care (Contract, Potential to Extend) Location: Greeley, CO Schedule: Monday–Friday, 7:00 AM–4:00 PM Contract Duration: July 17 – August 15, 2025 (with potential for extension or permanent hire)

Position Highlights:

• Full-time, weekday schedule – no nights, weekends, or call

• 30-day contract with possibility for extension or permanent placement

• Serve an established patient population of municipal employees and dependents

• Collaborative setting with access to nursing and administrative support staff

• On-site services include primary care, chronic disease management, wellness visits, immunizations and minor acute care

Key Responsibilities:

• Perform comprehensive health assessments and develop treatment plans

• Provide primary care services to patients ages 2 and up

• Conduct annual physicals, acute visit, and chronic care follow-ups

• Administer immunizations and perform minor procedures within scope

• Document encounters using EMR and coordinate care as needed

Qualifications:

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Colorado

• Minimum of 2 years of experience in a primary care, family medicine or urgent care setting preferred

• Current BLS certification

• Strong communication skills and a patient-centered approach

• Comfortable working autonomously in a fast-paced clinic environment
