We are seeing a Family Practice Nurse Practitioner (FNP) or Physician Assistant (PA) to join our family practice office located in Englewood, CO. Hampden Medical Group has been in Englewood for over 70 years providing family medicine for generations of our neighbors. Our practice has always been physician owned and family focused; we have patients who started with our practice when we opened our doors, and we are lucky to have many multi-generational families who call our practice home. HMG has been a part of many national advanced primary care reform programs over the years, including CPC, CPC+, and PCF, that strive to deliver evidence-based practice to our patients, improve patient outcomes, and move forward in the practice of medicine. Each provider at our office has their own medial assistant, and our APPs work closely with their supervising physicians to create a strong medical team. HMG is patient centered but also cognizant of the stress that primary care can put on our providers and staff, and we strive to make sure our employees can find a work/life balance. Additional information/requirements:

· NP: must be currently licensed as a Family Nurse Practitioner (FNP) in the State of Colorado and the license must be in good standing with no restrictions.

· PA: must be currently licensed as a Physician’s Assistant (PA) in the State of Colorado and the license must be in good standing with no restrictions.

· Must have a current DEA with full prescriptive license authority.

· Duties will include providing care to patients of all ages, both well exams and management of acute and chronic medical conditions

· Flexible schedule

· 2 days per week (Monday and Tuesday), with the option of 3 if desired

· 12-16 patients per day

· Shared call schedule between providers (low volume)

· $40,000-$42,000/year

Pay: $40,000.00 – $42,000.00 per year
