Job Description for Full Scope Primary Care Advanced Practice Provider (Family Nurse Practitioner, Physician Assistant) at AFC Urgent Care Denver

Position: Full-Time Advanced Practice Provider (APP)—Family Nurse Practitioner (FNP) or Physician Assistant (PA) with comprehensive primary care experience.

Overview: AFC Urgent Care Denver seeks experienced Advanced Practice Providers with a minimum of 2 years in with full scope primary care experience.

Pay Range: $108,000-$165,000, with a competitive bonus structure based on performance, including shift differentials for weekend work. The first 3 months offer 100% of bonus achievement.

Responsibilities:

Patient Assessment

  • Perform comprehensive health assessments, including obtaining medical histories and conducting physical examinations.
  • Evaluate and diagnose acute and chronic conditions.

Diagnosis and Treatment

  • Order, interpret, and follow up on diagnostic tests such as lab work, imaging studies, and screenings.
  • Develop and implement evidence-based treatment plans, including prescribing medications, therapies, and lifestyle modifications.

Preventive Care

  • Provide age-appropriate screenings, immunizations, and preventive health counseling.
  • Educate patients on managing risk factors for chronic diseases like diabetes, hypertension, and cardiovascular conditions.

Chronic Disease Management

  • Monitor and manage long-term conditions such as diabetes, asthma, COPD, and heart disease.
  • Coordinate care with specialists when needed.

Acute Care Management

  • Address urgent health concerns like infections, injuries, and exacerbations of chronic conditions.
  • Provide appropriate referrals when necessary.

Qualifications:

– Medical/Master’s degree with certification. (NP: BSN + MSN/DNP focused on a population (e.g., Family, Adult).

– Current, unrestricted Colorado medical license.

– Unrestricted Colorado DEA license to prescribe controlled substances.

– BLS or ACLS certification.

– PA: NCCPA certification (PANCE exam), NP: AANP, ANCC

Job Type: Full-time

Pay: $108,000.00 – $165,000.00 per year
