Full Job Description

STRIDE Community Health Center is a 501(c)(3) Federally Qualified Health Center (FQHC) with a 30-year history of providing primary care and other health-related services in a community-based setting. Established in 1989 in response to the primary health care needs of the medically indigent population in suburban Denver, STRIDE Community Health Center has the largest population base of any FQHC in Colorado and provides primary care, dental, pharmacy, behavioral health, health education, care coordination, patient navigation, pharmacy, and outreach and enrollment services in accordance with its mission.

General Purpose: In order to ensure efficient, quality health care services, the Nurse Practitioner or Physician Assistant will provide medical screenings and primary care to our patients.

Essential Duties/Responsibilities:

  • Provides direct health care services to patients.
  • Offers clinical consultation and arranges referrals to other providers and clinical staff.
  • Reviews lab and x-ray results of other providers patient care results.
  • Prescribes medications to patients, including counseling patients regarding the proper use and contraindications of medications as well as detect and diagnose adverse drug reactions.
  • Evaluates medical charts to determine appropriate patient care follow up and education regarding acute and chronic illnesses.
  • Accurately and completely charts information regarding patient interactions into the Electronic Health Record (EHR).
  • Participate in the STRIDE after-hours telephone call system.

Non-Essential Duties/Responsibilities

Attends all staff meetings and all other meetings as assigned.

Qualifications

A successful candidate will possess the following education or formal training:

Education or Formal Training:

  • Masters degree in nursing from an accredited Nurse Practitioner Program and/or Physician Assistant Program.
  • Family Practice Certification
  • Board certification.
  • CO License or eligible for licensure.

A successful candidate will possess experience:

Material and equipment directly used: Clinical equipment such as: Nebulizer, Pulse Oximeter, and EKG. Computer, printer, multi-line telephone systems, fax machine, copy machine, Microsoft Office Outlook, the internet and EHR. General medical equipment used for patient care.

Working environment/physical activities: High risk exposure to blood borne pathogens. Reliable personal transportation. Office environment within a clinical setting. Enters data into computer programs via computer, mouse and keyboard. Moves about the office environment and occasionally to other locations. Communicates information to others. Discerns/analyzes and synthesizes information from others to make decisions.

** A valid drivers license will be required for any position requiring driving between STRIDE locations. **

In an aim to help keep our patients, staff, and communities safe, STRIDE requires that all hired employees be fully COVID-19 vaccinated and to obtain an annual flu shot. STRIDE CHC employees are also required to be screened for tuberculosis and to complete a PPD test.
