Peak Med Wellness is a newly opened provider-directed wellness clinic in Fort Collins specializing in:

• Medical weight management
• Men’s & women’s hormone optimization
• Peptide therapies
• Wellness injections
• Red light therapy
• Advanced wellness services

We are building a clinic centered around patient relationships, thoughtful care, and creating an environment where providers can practice with autonomy, professionalism, and genuine connection with patients.

We are looking for a patient-focused NP or PA who values education, communication, and long-term patient success — not rushed visits or volume-based care.

This position begins PRN with strong potential to transition into a regular part-time schedule as the clinic continues to grow.

What You’ll Do

• Perform patient consultations and follow-up visits
• Review labs and develop treatment plans within Medical Director protocols
• Manage wellness, hormone, and weight management patients
• Educate patients on treatment options and ongoing care plans
• Work collaboratively with MAs and support staff
• Maintain accurate and timely charting
• Deliver an exceptional patient experience with professionalism and compassion

What We’re Looking For

• Experience in hormone therapy and/or medical weight management strongly preferred
• Comfortable interpreting labs and managing ongoing care plans
• Pellet insertion experience highly preferred
• Peptide, ED, hair restoration, or wellness medicine experience preferred
• Strong communication and relationship-building skills
• Comfortable in a collaborative, patient-centered environment
• Providers who enjoy helping shape clinic culture and contributing ideas

We welcome providers who have specific interests, specialties, or wellness passions they would like to help incorporate into the clinic as we continue building services and programs.

Requirements

• Active Colorado NP or PA license
• DEA license in good standing
• Ability to work under established Medical Director protocols
• Professional, reliable, and detail-oriented
• Must be available for at least two of the following shifts:
• Wednesdays 11 AM – 7 PM
• Thursdays 8 AM – 3 PM
• Fridays 8 AM – 3 PM
• Saturdays 8 AM – 12 PM
