NCH Health Partners, a division of Northern Colorado Hospitalists, is a physician-led, independently owned, mission-driven practice with a total of more than 80 providers caring for the rapidly growing Northern Colorado region.
Based in Fort Collins, we deliver a truly integrated care model, offering hospitalist care at UCHealth hospitals, post-acute services in SNFs, and primary care in ALFs, ILFs, and the community—all connected through one unified EMR for seamless patient management.

About the Role
This position plays a vital role in supporting patients in post-acute care SNFs and primary care in ALFs and ILFs. Essential functions include:
- Patient care that is focused on post-acute care services and adult medicine consultation
- Timely and accurate completion of medical records in accordance with applicable contracts and regulations
- Timely return of pages, calls, and correspondence from physicians, APPs or staff
- Maintaining current licensure and certification in good standing under the laws of the State of Colorado or as required by any federal or local agency
- Maintaining any required certifications and continuing education requirements
- Maintaining Facility and clinic privileges in good standing at the Facilities
- Practicing at all times in compliance with the current standards of medicine and in conformity with the prevailing standards of medicine
- Practicing at all times in a manner consistent with all principals of professional ethics and consistent with any and all Federal and State Health Care Laws and Regulation
An Exceptional Schedule and Work Environment
- Full-time, Monday – Friday
- Hours are flexible within the 8:00 – 5:00 window
- 14 – 15 patients per day to support budgetary targets at northern Colorado locations
- On call support 5pm onwards
- Ability to take at least 4 weeks off per year
- Weekends off except occasional on call shift
- Holidays off except occasional on call shift
- Strong MA support covering scheduling, in-basket review, and coordination of care

- Lower acuity compared to Hospitalist roles
- Reduced volumes compared to PCP clinics (fewer daily encounters than outpatient practices and allow more time for thoughtful care and decision-making).

Qualifications
- APP licensed to practice medicine in the state of Colorado and board certified or board eligible in his/her specialty

Pay: $107,000.00 – $117,000.00 per year
