Job description:
Position: Full-Time or Part-time Advanced Practice Provider (APP) Family Nurse Practitioner (FNP) or Physician Assistant (PA) with comprehensive primary care experience.
Overview: AFC Urgent Care Denver seeks experienced Advanced Practice Providers with a minimum of 2 years in full scope of urgent care and primary care experience. Candidates should have a strong bedside manner, enjoy patient interaction, and be able to work independently with a focus on patient care.
Pay Range: $108,000-$165,000, with a competitive bonus structure based on performance, including shift differentials for weekend work. The first 3 months offer 100% of bonus achievement.
Responsibilities:
Patient Assessment
- Conduct comprehensive patient evaluations, including detailed medical histories and physical examinations across all age groups.
- Assess and triage patients presenting with both acute and chronic conditions in a fast-paced clinical environment.
- Identify risk factors and early signs of disease through clinical judgment and evidence-based screening practices.
Diagnosis and Treatment
- Order, interpret, and follow up on diagnostic tests including laboratory work, imaging, and point-of-care testing.
- Formulate accurate diagnoses and develop evidence-based treatment plans.
- Prescribe medications, therapies, and lifestyle interventions in accordance with clinical guidelines and state regulations.
- Perform minor in-office procedures as clinically indicated.
Preventive Care
- Deliver age-appropriate preventive services such as annual physicals, immunizations, and health screenings.
- Provide patient education on disease prevention, wellness, and lifestyle modifications.
- Counsel patients managing risk factors related to chronic illnesses such as hypertension, diabetes, and cardiovascular disease.
Chronic Disease Management
- Serve as the primary provider for patients with chronic conditions including diabetes, asthma, COPD, and heart disease.
- Monitor disease progression, adjust treatment plans, and ensure continuity of care.
- Coordinate with specialists and other healthcare professionals to optimize patient outcomes.
- Educate patients on self-management strategies and medication adherence.
Acute Care Management
- Evaluate and treat urgent conditions such as infections, minor injuries, laceration repairs (suturing), incision and drainage (I&D), ingrown toenail removals and acute exacerbations of chronic diseases.
- Perform timely interventions and determine appropriate disposition (discharge vs. referral).
- Refer patients to higher levels of care or specialists when necessary.
Qualifications:
- Medical/Master’s degree with certification. (NP: BSN + MSN/DNP focused on a population (e.g., Family, Adult).
- PA: NCCPA certification (PANCE exam), NP: AANP, ANCC
- Current, unrestricted Colorado medical license.
- Unrestricted Colorado DEA license to prescribe controlled substances.
- BLS or ACLS certification.
- Experience in ER, urgent care, or fast-paced primary care.
