**Role:**
Nurse Practitioner, Advanced Practice Provider
**Responsibilities**
* Provide high-quality, equitable primary care to adult Medicare patients, focusing on patient satisfaction, health maintenance, and hospital avoidance.
* Conduct office visits for routine and acute issues, including annual wellness visits and health risk assessments.
* Coordinate care with physicians, specialists, testing facilities, and agencies to ensure comprehensive patient management.
* Lead population health initiatives, such as preventive screenings and health promotion activities.
* Assist with phone triage, patient education, and outreach efforts, including home visits when needed.
* Participate in promotional activities and other assigned duties, reporting to the Center Medical Director and collaborating with operational leadership.
**Qualifications & Skills**
* Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
* Active, non-probationary state Nurse Practitioner license and DEA license.
* National certification in Family, Adult-Gerontology, Adult, or Gerontological Nurse Practitioner.
* US work authorization.
* Preferred: primary care experience, understanding of HCC, ICD-10 coding, and health risk assessments; teamwork orientation; bilingual skills.
**Additional Details:**
Full-time role with a typical weekly schedule of 40 hours. Compensation ranges from $95,738 to $206,206 annually, depending on experience and location. Benefits include medical plans, 401(k), stock plans, wellness programs, flexible schedules, and additional support resources.
