Our patients choose to pay out of pocket for this level of care, even when insurance would otherwise cover a primary care visit. They do it because they want a provider who knows their story, has time for them, and treats them like a person instead of a chart to move through the day. That trade-off is the whole point of what we do, and it’s the standard every provider on our team is expected to meet.
The Role
We’re looking for a Nurse Practitioner or Physician Assistant to join our growing DPC practice on a part-time basis.
This isn’t a high-volume, insurance-driven clinic, and it isn’t a role for someone who needs a rigid protocol for every scenario. Patients come to us specifically because they want something different, and we need a provider who wants to deliver that difference: real relationships, real time, and care that flexes to fit the person in front of them.
About 60% of our current panel arrives at Lineage Wellness after completing 4–6 month intensive functional medicine deep-dive. These patients are usually already on a protocol that’s working. Your job isn’t to start from scratch, it’s to maintain what’s successful and keep an eye out for the next opportunity to fine-tune it as their goals and labs evolve.
Who You Are
This role is built for a specific kind of clinician. You’ll thrive here if:
- You see patients as people, not data points. You’re building a relationship over years, not processing a chief complaint and moving to the next room.
- You get why patients pay for this. You understand why someone would choose to pay a membership when insurance would cover a visit elsewhere, and you see it as your job to make that choice feel worth it every time.
- You think on your feet. You don’t need a flowchart to tell you what to do next. Protocols shift, patients don’t fit into boxes, and you’re comfortable making a sound clinical call in the gray areas rather than waiting for a black-and-white answer.
- You go find the answer. When a case doesn’t have an obvious playbook, you research it, ask a colleague, dig into the literature, or work it out, rather than needing someone to hand you a protocol.
- You’re a team player about coverage. With two providers in the office, sometimes a patient needs support during a window when you’re not the one on schedule. You’re glad to coordinate with your teammate so patients get a timely, thoughtful response, because that kind of teamwork is part of what makes the care here feel different.
- You’re energized by autonomy, not anxious about it. You’d rather have room to practice thoughtfully than a stack of rules to follow for their own sake.
To be direct: this role is not a fit for a highly linear, checklist-driven thinker who wants one correct answer for every situation. Medicine in a DPC model is more art than formula, and we need someone who’s genuinely energized by that, not someone who tolerates it.
Responsibilities
Clinical Care
- Provide comprehensive primary care across the lifespan (teens through seniors), including physicals, chronic disease management, and acute visits.
- Confidently manage men’s and women’s hormone therapy, peptide therapy, and other advanced or alternative protocols, alongside standard primary care needs.
- Order and interpret advanced labs, imaging, and diagnostics, and build supplementation and lifestyle protocols tailored to the individual.
- Run efficient, well-structured 45-minute visits that make time for both reviewing lab and testing data and building the forward-looking care plan, without running long or shortchanging either part.
- Take over care for patients transitioning from Trivida’s intensive functional medicine program, maintaining protocols that are already working and identifying opportunities to fine-tune them further.
- Adapt care plans as new information, research, or patient response comes in, rather than defaulting to a fixed protocol.
Relationship & Membership Guidance
- Build long-term, trust-based relationships that make patients feel known, not managed.
- Help patients understand and choose the right membership tier, testing, and protocol options for their goals, in a way that feels like guidance, not a pitch.
- Take the time to listen, educate, and empower patients to take ownership of their health.
Communication & Flexibility
- Respond to patient messages between visits with sound judgment, including occasional light-touch communication outside standard clinic hours.
- Exercise independent clinical judgment in nuanced or non-standard situations without needing constant oversight.
Collaboration & Growth
- Work closely with the Trivida Functional Medicine team for coordinated, comprehensive care.
- Contribute to clinic systems, membership structure, and processes as the practice grows.
- Stay current with evolving research in conventional, functional, and integrative medicine.
Qualifications
Must-Have
- Licensed NP or PA in good standing, with Colorado licensure in place.
- Minimum 2 years of clinical experience; primary care experience strongly preferred.
- Demonstrated confidence in men’s and women’s HRT, peptide therapy, and alternative or integrative protocols, in addition to standard chronic disease and primary care management.
- Comfort making independent clinical decisions without needing a rigid protocol or constant sign-off.
- A track record of researching and problem-solving independently rather than needing a predetermined answer.
Nice-to-Have
- Experience in functional or integrative medicine.
- Prior experience in a DPC or membership-based practice.
- Comfort discussing cost, value, and membership decisions directly with patients.
Schedule & Location
- Part-time, with a flexible schedule built around the practice’s and provider’s needs. Options may include three 10-hour days, four 8-hour days, or a similar structure.
- In-person role based in Denver Tech Center / Centennial, with virtual visits available for patient flexibility.
- Occasional light-touch patient communication outside scheduled clinic hours; this is not a formal on-call structure, and we’re intentional about keeping it manageable.
Compensation & Benefits
- Hourly rate: $60–$70 per hour.
- Performance-based bonuses tied to patient retention and practice growth.
- Benefits available once weekly hours reach 30 or more.
- Long-term growth and partnership potential within two years.
- No insurance billing, quotas, or production-based pressure. Systems and operations are already in place so you can focus on patients.
