Nurse Practitioner/Physician Assistant Job Description

OverviewWe are seeking a skilled and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our dedicated wound care team. The successful candidate will provide specialized wound care services to patients in two nursing home facilities, twice a week.

Duties:

• Conduct rounds at designated nursing homes every Wednesday at 7:30 AM to deliver specialized wound care.
• Perform wound debridement procedures and recommend appropriate wound dressing based on clinical knowledge.
• Provide surgical debridement and dressing recommendations as needed.
• Maintain a consistent schedule of rounds and procedures every Wednesday.
• Accurately document detailed notes on patient conditions and wound measurements after each round.
• Follow up with discharged patients in assisted living or home settings to continue wound care management.
• Report to the physician supervisor on a biweekly basis.
• Participate in monthly rounds with the physician supervisor.
• Collaborate with nursing home administrators to identify and implement improvement strategies.

Qualifications:

• Valid PA or NP license in the state of California.
• Previous experience in wound care is highly desirable.
• Competency in performing wound debridements.
• Proficiency in using Electronic Medical Records (EMR) systems for patient documentation and billing, as well as familiarity with nursing home EMR systems.
• Excellent communication and interpersonal skills.
• Ability to work independently and make critical decisions in patient care.
• Willingness to travel to multiple nursing home locations.

Compensation:

Pay rate is commensurate with experience.
