Overview
Cucamonga Valley Medical Group embodies excellence in all that we do to provide compassionate care to our patients. We currently operate in 6 locations, N. Fontana, Central Fontana, Eastvale, Upland, Rancho Cucamonga, and Ontario-Chino. We are looking for compassionate NP/PA Practitioners to join our team of highly professional providers. Our Advanced Practitioners are responsible for providing comprehensive, quality care for CVMG’s patient population by treating, diagnosing, and educating CVMG’s patients on all matters about their health and well-being.
The NP/PA Practitioner will manage both episodic and chronic conditions while maintaining an emphasis on wellness, health education, and preventative medicine. Our Practitioners are expected to see an average of 24 patients per day, ranging in age from newborns to geriatrics, to complete charts effectively and promptly, to continually review and analyze test results, messages, refills, etc., and to exhibit leadership in managing patient care within the clinic environment. Practitioners at CVMG actively engage in continuing education to maintain and grow their professional knowledge and leadership skills. Overall, they are expected to demonstrate CVMG’s Core Culture in their interactions with patients, staff, and peers.

Requirements:
- Current California Nurse Practitioner or Physician Assistant License
- California DEA Certificate
- Current NPI
- Current BLS
