Job Title: Advance Practice Providers

We are excited to announce new opportunities for Advanced Practice Provider (APP) – Nurse Practitioner (NP) or Physician Assistant (PA)—to join our primary care clinic in Sunnyvale. Become an integral part of our close-knit primary care team. We are looking for an APP who thrives in a collaborative environment, driving our commitment to exceptional patient care and wellness.

  • Be empowered to practice at the top of your license, managing a diverse patient panel while benefiting from a strong, supportive relationship with our supervising physician.
  • Delive comprehensive health services and nurture long-term patient relationships.
  • Dive into diverse clinical cases where you’ll expertly diagnose conditions, prescribe crucial medications, and order essential diagnostic studies.
  • Optimize clinical workflows to enhance patient experience and practice efficiency.

What you’ll do

  • Conduct thorough patient assessments and develop individualized care plans.
  • Perform diagnostic evaluations and interpret medical tests to guide treatment decisions.
  • Effectively use EMR features to optimize clinical workflows and improve patient care efficiency
  • Educate patients on health management strategies, including nutrition and chronic disease management.
  • Participate in quality initiatives to enhance patient outcomes.

Basic qualifications

  • Current and unrestricted California license as a Nurse Practitioner (NP) or Physician Assistant (PA).
  • National board certification appropriate to your role (e.g., AANP, ANCC, or NCCPA).
  • Active DEA certificate.

Job Types: Part-time, Contract

Pay: $30.00 – $32.00 per hour

Expected hours: 16 – 24 per week
