Healthly is recruiting Nurse Practitioner or Physician Assistant who is double board certified in Lifestyle Medicine and a primary care specialty. You will take care of a panel of Medicare and concierge patients, providing full spectrum primary care services, and helping to coordinate the Lifestyle Medicine team that includes health coaches, dietitians, personal trainers and social workers; and incorporates groups, ITLC’s, lifestyle medicine clinical protocols, and numerous other resources that allow lifestyle medicine to really happen.

Majority of our patients have chronic conditions and are typically older adults.

Role Description

As a Nurse Practitioner or Physician Assistant, you will care for your own panel of patients as well as support 1-2 physicians in conducting routine screenings, chart reviews, and urgent acute care for patients in the physicians’ panels. You will lead a team of allied health professionals and medical assistants. You will also work closely with other providers in daily, weekly and monthly huddles.

Responsibilities include:

  • Conduct in-person or virtual visits typically in 30 minute appointment windows, with strong focus on guiding patients on developing their own plans for health
  • Population health initiatives focused on proactive strategies to prevent problems before they arise
  • Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation
  • Understand and apply HCC documentation and ICD-10 coding
  • Care coordination with other providers, specialists, testing facilities, agencies
  • Phone triage and outreach
  • Patient education
  • Practice promotional activities, such as health seminars
  • Home visits as needed
  • Other duties as assigned

Language

Fluent Spanish is a strong plus.

Compensation

  • $125,000 per year base salary (pro-rated if part-time)
  • Opportunity for bonus of $30,000 to $50,000 per year based on patient panel size, clinical outcomes and quality performance
