Full Job Description
Vivo HealthStaff is seeking a full-time Nurse Practitioner or Physician Assistant for a Healthcare Organization for Seniors in San Marcos, California.
Responsibilities:
- Provide primary care and acute illness treatment for participants in collaboration with PACE Physicians.
- Conduct comprehensive assessments and collaborate with physicians to develop care plans.
- Manage participant care in the nursing home and coordinate 24-hour care delivery.
- Review outcomes and utilization data and make policy and procedural improvements.
- Maintain confidentiality of company procedures and information.
Education and Certification:
- NP/PA CA State License.
- Certification as a Nurse Practitioner/Physician Assistant with prescriptive authority
- Current Basic Life Support (BLS) training.
- Current driver’s license and proof of auto insurance.
Skills & Experience:
- Minimum of 1 year of work experience as an NP/PA doing adult chronic disease management and functioning in a PCP role with or without MD oversight. Specific work experience with the frail, elderly population is preferred.
- Current knowledge of chronic care/geriatric issues and best practices, experience with deprescribing, and palliative care would be a bonus.
- Ability to build strong working relationships with other providers and an interdisciplinary team (IDT).
- Creative and enthusiastic commitment to the “triple aim” – dedication to improving the experience, health, and value for PACE participants.
- Ability to process information accurately and timely, and able to learn new EMR and health tech and communication platforms quickly.
- Excellent clinical, organizational, and communication skills in settings with frail, elderly participants, and their families. Experience with having difficult goals of care and “What Matters” conversations would be preferred.
- Working knowledge of PACE regulations would be a bonus but not required.
Salary: 140,000-147,000
