What You’ll Do:
✔️ Provide in-home or on-site care at private residences, senior living communities, and long-term care settings ✔️ Manage chronic conditions, preventive care, and mental health needs for an aging population ✔️ Collaborate with interdisciplinary teams to deliver whole-person, senior-focused care ✔️ Build trusted relationships through empathy, consistency, and patient education ✔️ Support care access and equity by removing barriers for vulnerable seniors

Building partnerships with senior communities, clinics, and referral partners Representing our mobile healthcare model to patients, families, and providers Supporting community engagement alongside our outreach and marketing teams
