Nurse Practitioner/Physician Assistant

Full Job Description

Vivo HealthStaff is recruiting Nurse Practitioners in San Jose, California. This is a permanent full-time position.

Details:

• Provide comprehensive healthcare to patients with complex and chronic care needs.
• Job is hybrid ( some days remote and some days doing home visits)
• Conduct initial and annual face-to-face patient visits at home; will be seeing 5-7 patients per day; 5 if doing home visits and 7 if tele visits
• Perform health risk assessments, including medical history review, symptom evaluation, and physical examinations.
• Develop care plans for chronic disease management.
• Collaborate with a multidisciplinary team to ensure quality patient care.

Requirements:

• Master’s degree from an accredited NP Program or PA program
• One (1) year of prior clinical or home care experience
• Previous EMR experience preferred
• Active California state Nurse Practitioner or Physician Assistant license
• Active RN License and Furnishing number (Must, upon hire) NP only
• Active Nurse Practitioner Board Certification/Physician Assistant Certification
• NPI Number, DEA, Valid BLS
• Valid California driver’s license and current automobile insurance
• Experience in the care of older adult (geriatric) patients preferred
• Excellent administrative, organizational, and communication skills required
