Your Responsibilities

  • Deliver personalized, high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
  • Conduct comprehensive assessments and develop tailored care plans for older adults.
  • Manage chronic conditions with skill and empathy for patients and families.
  • Collaborate with cross-functional partners—including palliative, psych, and neuro—to deliver coordinated, whole-person care.
  • Build trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
  • Maintain consistency in documentation, follow-through, and patient communication.

What We’re Looking For

  • Active, unencumbered Physician Assistant license in the state you will be practicing.
  • Minimum of 1–2 years of clinical experience (geriatrics or primary care preferred).
  • Leadership potential or experience: Ideally, you’ve managed a panel successfully and are ready to grow into leadership roles.
  • Experience in home-based care, SNF, or ALF settings strongly preferred.
  • Compassionate, patient-centered approach and strong clinical judgment.
  • Valid driver’s license and reliable transportation.
