Physician Assistant / Nurse Practitioner — Field Medicine Program, San Mateo County
About the Program
Starfield Primary Care is launching a new field medicine initiative serving high-risk patients in San Mateo County. This innovative program integrates mobile, field-based medical care with Enhanced Care Management (ECM) services to deliver comprehensive, community-embedded support for vulnerable populations.
We are contracted with the Health Plan of San Mateo and Medicare to provide both medical and ECM services to individuals facing complex medical and social challenges. Our early team includes a lead case manager deeply familiar with the San Mateo community and a part-time nurse practitioner with strong local connections and extensive experience in field-based care.
We are now seeking a dedicated Physician Assistant or Nurse Practitioner to join this pioneering effort and help grow and expand the program.
Role Overview
In this position, you will provide mobile medical services in a variety of community-based settings, including:
- Supportive housing sites for formerly homeless or housing-insecure clients
- Residential care facilities for the elderly (RCFEs) and other high-risk seniors
- Homeless shelters
- Street-medicine outreach for individuals who remain unsheltered
You’ll work collaboratively with a case management team, a seasoned community-based nurse practitioner, and a medical director who provides mentorship and clinical oversight. Together, you’ll form an interdisciplinary team addressing both the medical and social determinants of health for vulnerable populations across San Mateo County.

Key Responsibilities
- Conduct field-based and mobile medical visits in community, residential, and shelter settings
- Collaborate with ECM case managers and social service partners to develop and implement care plans
- Provide preventive, acute, and chronic disease management for complex, high-risk patients
- Participate in team case reviews and multidisciplinary rounds
- Document encounters thoroughly and communicate with the care team to ensure continuity
- Partner with leadership to identify and engage new community sites (e.g., supported housing, RCFEs, shelters) to expand access to services
Preferred Qualifications
- Current California PA-C or NP licensure
- Experience in primary care, community health, or mobile medicine preferred
- Passion for working with underserved or high-risk populations
- Strong interpersonal and teamwork skills
- Comfort working independently in the field with supportive remote supervision
- Willingness to engage in program development and community outreach
Schedule & Compensation
- Preferred availability: at least three days per week for field-based work
- Protected administrative time for documentation and care coordination

Job Types: Full-time, Part-time, Contract
Pay: $130,000.00 – $160,000.00 per year
Expected hours: 20 – 40 per week
