Job Summary: As an Outpatient Post-Acute Wound Care Nurse Practitioner/Physician Assistant, you will play a critical role in the assessment, treatment, and management of patients with acute and chronic wounds. You will work collaboratively with our multidisciplinary team to provide comprehensive wound care services to our outpatient population within the Orange County. Your expertise and dedication will be instrumental in promoting wound healing, preventing complications, and improving patients’ quality of life. Key Responsibilities:
- Wound Assessment: Perform thorough wound assessments, including wound dimensions, tissue viability, infection status, and associated symptoms.
- Treatment Planning: Develop individualized wound care plans based on assessment findings, evidence-based practices, and patient goals.
- Wound Dressing and Procedures: Administer wound dressings, debridement, and other advanced wound care procedures as needed.
- Patient Education: Educate patients and their families on wound care techniques, prevention strategies, and self-care practices to promote healing.
- Collaborative Care: Work closely with physicians, nurses, physical therapists, and other healthcare professionals to coordinate care and ensure optimal patient outcomes.
- Documentation: Maintain accurate and up-to-date patient records, including wound assessments, treatment plans, progress notes, and outcomes.
- Quality Improvement: Participate in quality improvement initiatives to enhance the effectiveness and efficiency of wound care services.
Patient Advocacy: Advocate for patients’ needs and concerns, ensuring they receive the highest quality of care and support.
Qualifications:
- Education: Master’s degree in Nursing with certification as a Nurse Practitioner (NP). Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) preferred.
- Licensure: Current state NP license and DEA registration. Advanced Practice Registered Nurse (APRN) certification.
- Experience: Minimum of 1 year of clinical experience as a Nurse Practitioner, with a focus on wound care in an outpatient or post-acute care setting.
- Wound Care Certification: Wound, Ostomy, and Continence Nurse (WOCN) certification or wound care specialization preferred.
- Communication Skills: Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
