OverviewHouseCall MD is seeking a Nurse Practitioner to provide high-quality, patient-centered care in home-based and facility settings.

Key Responsibilities
- Provide comprehensive primary care to patients in their homes and facilities
- Develop and implement personalized treatment plans
- Review and utilize medical records to guide clinical decisions
- Communicate clearly with patients families and care teams
- Educate patients on conditions treatments and care plans
- Deliver compassionate holistic care aligned with patient goals
Qualifications
Required
- Active Nurse Practitioner license
- Minimum 2 years of primary care experience
- Strong clinical knowledge and decision-making skills
- Excellent communication and interpersonal skills
Preferred
- Experience in home health geriatrics or palliative care
- Strong ability to build relationships with patients and families
Work Schedule and Flexibility
- Flexible schedule typically between 7AM and 6PM
- Average of 40 hours per week
- No formal on-call requirement
- Occasional urgent patient-related communication as needed
