Job Summary
The Community Clinic Physician Assistant or Family Nurse Practitioner provides comprehensive, patient-centered primary care services to patients of all ages in a nonprofit Title 22 community clinic setting. This role is integral to advancing our mission of delivering accessible, high-quality healthcare to underserved populations.
As a nonprofit community clinic, we are eligible for enhanced fee-for-service reimbursement under applicable state and federal programs, allowing providers to focus on quality, continuity, and preventive care rather than volume-driven medicine. The ideal candidate is clinically strong, mission-driven, and comfortable practicing across the lifespan in a collaborative, supportive outpatient environment.
This position offers the opportunity to work closely with physicians, nursing staff, behavioral health providers, and care coordinators to improve health outcomes, reduce disparities, and strengthen community health.
Key ResponsibilitiesClinical Care
- Provide comprehensive primary care services to pediatric, adult, and geriatric patients.
- Conduct complete patient assessments including medical history, physical examinations, and preventive screenings.
- Diagnose and manage acute and chronic conditions in accordance with evidence-based clinical guidelines.
- Prescribe medications and order diagnostic tests consistent with scope of practice and clinic protocols.
- Perform office-based procedures such as suturing, wound care, incision and drainage, injections, IV starts, catheterization, and basic splinting.
- Deliver age-appropriate preventive care, health maintenance, and chronic disease management.
Care Coordination & Continuity
- Coordinate referrals to specialists and ancillary services as needed.
- Collaborate with care management teams to support high-risk and complex patients.
- Provide patient education on disease prevention, medication adherence, nutrition, lifestyle modification, and self-management.
- Participate in transitional care management and follow-up services when applicable.
Documentation & Compliance
- Document all encounters accurately and timely using the clinic’s EHR system.
- Ensure compliance with Title 22 community clinic regulations, clinical protocols, and payer requirements.
- Support quality improvement initiatives, audits, and performance measures aligned with state and federal standards.
Access & Innovation
- Provide telehealth services as appropriate to expand access to care.
- Participate in outreach, population health initiatives, and community-based programs.
Required Skills & QualificationsClinical Skills
- Strong foundation in primary care across the lifespan.
- Proficiency in common outpatient procedures and clinical decision-making.
- Knowledge of chronic disease management, preventive care, and acute care treatment.
- Understanding of infection control, sterile technique, and patient safety standards.
Technical & Administrative
- Experience with EHR systems such as Epic, eClinicalWorks, Athenahealth, or similar platforms.
- Familiarity with CPT and ICD-10 coding for accurate documentation and reimbursement.
- Ability to manage patient flow efficiently in a community clinic setting.
Communication & Professionalism
- Excellent interpersonal and communication skills with patients, families, and care teams.
- Cultural humility and sensitivity to diverse populations and community needs.
- Ability to work independently while collaborating within a multidisciplinary team.
Preferred Qualifications
- Prior experience in a community clinic, FQHC, or nonprofit healthcare setting.
- Experience caring for underserved or Medi-Cal populations.
- Background in behavioral health integration, care coordination, or population health initiatives.

Pay: $55.00 – $70.00 per hour
