Job Title: Primary Care Nurse Practitioner/Physician AssistantLocation: Los Angeles, CASetting: Home HealthTerm: 3 months with option to extendShift: 40 hours per week guaranteed.Typically, 8:00AM-5:00PM Monday through Friday. Flexibility required for some early or weekend shifts.Compensation: $125-130/hour
Astrya Global, a San Diego based medical staffing agency, is hiring a Primary Care Nurse Practitioner/Physician Assistant for a Home Health Pilot Program. This position is a 3-month locum tenens contract with the ability to extend.

Qualifications of a Primary Care Nurse Practitioner/Physician Assistant:
- Current, unencumbered California NP or PA license
- BLS certification (American Hospital Association)
- Spanish-speaking highly preferred
- Experience with EPIC
- 1 year of home health experience in the last 3 years required
Duties/Responsibilities of a Primary Care Nurse Practitioner/Physician Assistant:
- Assist with the Home Health Pilot Program, conducting evaluations, outreach, and home visits
- Provide virtual patient encounters as needed
- Conduct 1-2 home health visits per day within the West Los Angeles service area
- Maintain accurate documentation within EPIC
- Demonstrate flexibility and adaptability to patient and program needs
