Job Title: Street Medicine Physician Assistant Location: LA Metro 90033 Department: Street Medicine Program Schedule: Full-time | Field-based 3 days/week | Admin & outreach mix Pay Range: $150,000 – $165,000
Overview:
Join a mission-driven Street Medicine team providing essential care to unhoused individuals across Los Angeles. As a Physician Assistant, you’ll bring care directly to patients in shelters, encampments, and permanent supportive housing sites. This is a high-impact role ideal for a provider passionate about health equity and street-based outreach.
Key Responsibilities:

Deliver compassionate care including chronic disease management, wound care, diagnostics, and medication refills.

Conduct health assessments and administer preventive services such as immunizations and screenings.

Travel with the mobile unit or on foot to assigned service areas.

Collaborate with social workers and community health workers to address social determinants of health.

Maintain accurate, timely documentation in the EHR.

Support community events and build long-term trust with vulnerable populations.
Schedule Highlights:

Monday: Admin + PM fieldwork
-
Tuesday/Wednesday: AM at emergency shelters (8am-12pm), other outreach as assigned

Thursday: Permanent Supportive Housing visits or clinic-based care

Friday: AM admin, PM in field
-
Varying start times (earliest ~7:30am)
Qualifications:
-
Active California PA license
-
2+ years in emergency medicine, community health, urgent care, or family medicine

Skilled in managing chronic and acute conditions in non-traditional settings
-
Comfortable performing wound care and minor procedures
-
Deep understanding of the unique health challenges of homeless populations

Spanish bilingual strongly preferred
Additional Requirements:

Must pass a background check

Must be physically able to work outdoors and navigate uneven terrain

Ability to lift up to 30 lbs and perform hands-on clinical work in the field
