Job Title: Certified Nurse Practitioner or Physician Assistant
Status: Full Time
Location: Remote (Western US Based): California and Washington States Licensed
Bi-lingual Spanish (Required)

Who You Are: You will align with our healthcare team as a highly dedicated Nurse Practitioner or Physician Assistant who’s comfortable with technology and enjoys working remotely. As a rapidly growing digital health company, Guardiant Health is looking to work with practitioners who are passionate about combining medical, mental, social, and clinical care with technology to deliver personalized healthcare in a team-based environment.
As a member of an interdisciplinary care team, you will effectively collaborate with a supervising physician to deliver remote clinical services.
Who We Are: Guardiant Health offers virtual medical visits to individuals in the comfort and convenience of their home, or at their primary care providers office, with the primary goal and objective of providing best- in-class patient care, preventive health assessments and ensuring all required care is in place to ensure quality performance measures are being met. This work is vital in reducing avoidable hospitalizations, and decreasing unnecessary emergency department utilization.
Our overarching goal is to provide personalized care by empowering and guiding our members to take control of their health.
What You’ll Do: Your essential tasks may include conducting technology-enabled; remote medical wellness visits; facilitating quality measures; supporting chronic condition management; and providing advanced primary care that may be needed.
What We Require:
- Current, unrestricted RN/NP/PA California and Washington state license, DEA License, and privileges with the State board
- Graduate Degree from an accredited Master’s Program in Nursing (MSN) or PA (MPAS) program
- Board Certification through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) or Physician Assistant National Certifying Exam (PANCE)
- 3+ years of post-graduate experience working as a Nurse Practitioner or Physician Assistant in a virtual or in-person environment.
- 3+ years of practitioner experience in Medical Primary Care, in concert with Behavioral Health, Community and Social Determinates of Health or similar; and advanced knowledge of and experience with symptom management
- Ability to use technology to include conducting virtual visits via phone or computer
- Current Basic Life Support certification
