Nurse Practitioner / Physician Assistant (NP/PA)
Location: Serving Los Angeles and Orange CountySchedule: Full-time or Part-time | Flexible Schedule | Work at Your Own Pace
About Us
We are a patient-centered healthcare organization providing high-quality, compassionate care to individuals in home care facilities, nursing homes, assisted living facilities, hospice, and private residences throughout Los Angeles and Orange County. We are seeking dedicated Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our growing team.
Responsibilities
- Conduct patient visits in various care settings, including homes, nursing facilities, assisted living, and hospice.
- Assess, evaluate, and manage wounds, creating individualized care plans for wound treatment.
- Review patient medical records and perform physical examinations.
- Order, interpret, and review diagnostic tests (e.g., blood tests, imaging).
- Diagnose and manage acute and chronic illnesses.
- Prescribe and manage medications in accordance with current clinical guidelines.
- Maintain accurate and up-to-date patient documentation and schedules.
- Educate patients and families on disease prevention, health maintenance, and medication use.
- Promote overall wellness and coordinate with multidisciplinary care teams to ensure optimal outcomes.
Requirements
- Dependable, professional, and detail-oriented.
- Flexible and adaptable to varying patient needs and schedules.
- Compassionate, patient, and calm under pressure.
Qualifications
- Active and unrestricted California Nurse Practitioner or Physician Assistant license.
- Current DEA number required.
- Proven clinical experience in a hospital, clinic, ER, or similar setting.
- Reliable transportation and proof of auto insurance.
- Excellent communication and interpersonal skills.
- Computer literate (EHR experience preferred).
