Nurse Practitioner or Physician Assistant – Home-Based Primary Care Location: Pasadena, Glendale, Arcadia, Beverly Hills, Downey, East Los Angeles City, CA Job Type: Full-time or Part-time Compensation: $150,000 – $300,000 per year (based on experience and productivity)
Access Healthcare Associates is hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to join our mobile primary care team in the Los Angeles area, serving seniors in Pasadena, Glendale, Arcadia, Beverly Hills, Downey, and East Los Angeles City. This role is ideal for advanced practice providers who want autonomy, flexibility, and meaningful connection with patients—outside of the clinic setting.
Founded in 2007, Access Healthcare Associates is a physician-led mobile medical practice providing high-quality home-based primary care to seniors. Our care model is fee-for-service, focused on relationship-driven, comprehensive care delivered where patients live—whether in private homes, assisted living communities, or skilled nursing facilities.
Key Responsibilities:
- Conduct in-home visits to provide primary care services to seniors in residential and community-based settings
- Complete detailed health assessments, manage chronic and acute conditions, and develop personalized care plans
- Coordinate with specialists, ancillary services, and caregivers to deliver well-rounded, patient-centered care
- Document all patient encounters using a secure EMR platform
- Operate independently in the field while supported by a clinical and administrative leadership team
Qualifications:
- Active and unrestricted California NP or PA license
- Board certification (AANP, ANCC, or NCCPA)
- Experience in geriatrics, internal medicine, family practice, or home-based care preferred
- Valid driver’s license and reliable transportation
- Strong clinical decision-making skills, time management, and professionalism
- Passion for delivering high-quality care to older adults in their home environments

- Competitive earnings in a fee-for-service model with total annual potential between $150,000 to $300,000
