Job Title: Physician Assistant (PA) / Nurse Practitioner (NP) – Wound Care & G-Tube Specialist
Location: South Bay, Los Angeles (Home Health & Skilled Nursing Facility Settings)Coverage Area: Primarily south of the 110 and west of the 710 — flexibility may be required outside this area as needed.Schedule: Full-Time or Part-Time (Weekly SNF Rounds + On-Call for Emergent G-Tube Replacements)
About the Role
We are seeking an experienced and self-motivated PA or NP to join our advanced wound care and G-tube management team. This position focuses on chronic wound management and G-tube replacements across home health and SNF settings in the South Bay area.
The ideal candidate is an independent, accountable, and clinically skilled provider who thrives in a field-based role, providing comprehensive wound care, performing sharp and ultrasonic debridements, and managing routine and emergent G-tube replacements.
Key Responsibilities
- Provide chronic wound care and perform wound assessments, treatment planning, and follow-ups in home health and SNF environments.
- Perform sharp and ultrasonic wound debridements (training provided).
- Conduct routine and emergent G-tube replacements safely and efficiently.
- Collaborate closely with physicians, RNs/LVNs, and facility staff to coordinate patient care.
- Participate in weekly SNF rounds for wound management.
- Educate staff and patients on wound and ostomy care best practices.
- Complete accurate, timely, and compliant documentation in EMR systems.
- Demonstrate responsiveness, professionalism, and strong communication with all members of the care team.
- Travel between patient locations within the South Bay region (driving required).
Qualifications
- Licensed Physician Assistant (PA-C) or Nurse Practitioner (NP) in the State of California.
- Current National Certification required (must be verified prior to hire).
- Minimum 2 years of experience in wound care, debridement, or chronic wound management required.
- Proven experience performing G-tube placements/replacements is mandatory.
- Preferred: Certified Wound Specialist (CWS) or Wound Care Certified Nurse (WCON) — or willingness to attain certification within 1 year of employment.
- Prior experience in home health and/or SNF settings strongly preferred.
- Excellent clinical judgment and communication skills.
- Demonstrated autonomy, accountability, and reliability in independent practice.
- Strong teamwork skills and ability to collaborate effectively with multidisciplinary staff.
- Must have a valid driver’s license, reliable transportation, and proof of insurance.
- Background check (including driving record) will be required prior to hire.

Pay: $120,000.00 – $200,000.00 per year
