We have a great opportunity for an experienced Family Medicine APP Nurse Practitioner or Physician Assistant in our growing community healthcare organization! Must have minimum 2 years APP Clinical experience for consideration.

We have a strong preference for a clinician who is bilingual in English & Spanish for this position in our new Costa Mesa clinic. Hours of operation are Mon – Fri 8am – 8pm with Saturdays 8am – 5pm. Shifts are:
Mon – Fri 8am – 5pm OR Mon – Fri 12pm – 8pm and rotating Saturdays 8am – 5pm

JOB SUMMARY:
This Clinician, Nurse Practitioner (NP) or Physician Assistant (PA-C) uses clinical skills to provide comprehensive medical services for patients, regardless of age or sex within scope of training. Based on Credentialing and Privileging may manage all age populations effectively in a community health center clinic setting. Works closely and effectively with both clinical staff and front office staff as well as other clinicians as needed to maintain effective patient management and efficient workflow. Enters data proficiently in electronic medical record system and follows all protocols for safe, quality care. The Clinician Nurse Practitioner (NP) or Physician Assistant (PA-C) works in the team-based model of care.
RESPONSIBILITIES:
- Manages the care of patients in collaboration with other health professionals and members of the health care team in order to make decisions about treatment and attain health goals.
- Provides Services which include: Preventative care, immunizations, primary care and treatment, health education, prescribing of medications, ordering and interpreting lab testing, physical examinations, STD screenings, Vision/hearing, women’s health, family planning, prenatal services, and nutrition.
- Works to ensure productivity and quality goals are met.
- Provides high quality patient care at any site to patients of all ages as per privileging.
- Work within clinical privileges that are delineated either through a delegation of services agreement or Scope of Practice agreement.
- Coordinates and manages the care of patients and ensures that they receive optimal care, including acute illness management, chronic disease management, primary care and preventive care.
- Performs any and all clinic procedures in accordance with training, licensure and as allowed per Credentialing and Privileging.
- Follows established departmental policies, procedures and objectives, continuous quality improvement objectives, and other control standards.
- Interfaces with the entire clinical team to engage in continuous quality improvement and development of clinic-based services.
- Works at one or more sites managing patients and completing triage duties as required.
- May provide tele/virtual health visits
- Works closely with, RN, LVN, medical assistants and other clinic support team members.
- Completes referrals to other clinical organizations and providers.
- Follows up with pharmacy, laboratory orders and results.
- Updates medical records timely using the electronic health record system. Completes all required billing and coding documentation no later than 48 hours after encounter as per Share Ourselves policy.
- Ensures timely phone follow up is made with all contacts.
- Attends team and clinic meetings, including morning “team huddles”
- Expected to participate in provider on-call duties.
- Works additional hours and completes other duties as assigned
QUALIFICATIONS:
- Desire to work with an ethnically diverse all age population.
- Knowledge of the Patient Centered Medical Home (PCMH) model of care.
- Ability to work successfully and respectfully with patients and clients of diverse backgrounds and cultures; mentally ill, drug and alcohol abusers, and those experiencing homelessness.
- Ability to establish work priorities to meet established deadlines
- Ability to communicate effectively both orally and in writing
- Basic knowledge of local, state and federal regulations relating to care of patients in a clinic setting; and prior experience with county, state and federal programs such as CHDP, FPACT, EWC, Medi-Cal is preferred.
- Must be able to adhere to all clinic related policies and procedures including confidentiality and HIPAA.
- Maintains superior expertise and completes continuing education requirements timely
EDUCATION and / or EXPERIENCE:
- BSN and MSN Graduate from an accredited Nurse Practitioner Program or Physician Assistant Program.
- CA Nurse Practitioner or Physician Assistant licensure unrestricted
- Current DEA license unrestricted
- Current BLS certificate.
- Minimum Two (2) years of family medicine clinical practitioner experience
- Experience working with the Managed Care Medi-Cal and Medicare populations.
- Experience in direct outpatient care, management methods and practices in a community clinic or ambulatory care setting.
- Experience working with FQHC populations helpful and knowledge of health disparities preferred.
- Proficient clinical skills and ability to multi-task
- Proficient in English for both verbal and written communication, including use of basic medical terminology.
- Bilingual in Spanish is strongly preferred.
COMPUTER & EQUIPMENT SKILLS:
- Knowledge of Electronic Health Records Systems – develop proficiency within 3 months of training. Epic Experience preferred.
- Uses Word, Excel, Outlook and Internet software proficiently
- General knowledge of computer operations, proficiently uses a keyboard
- Familiar with medical diagnostic lab equipment
- Qualified user of primary care medical equipment and devices
