Full Job Description

POSITION DESCRIPTION:

Under the general direction of the Health and Wellness Center Manager and supervising physician, this position:

  • Exercises autonomy in decision making;
  • Provides a broad range of diagnostic and therapeutic services for patient’s primary, routine and/or acute medical needs through in-person, telephonic and video-based appointments;
  • May supervise support staff in a remote clinical setting; and
  • May have responsibility for education, research, and administrative services.

EXAMPLE OF DUTIES: [May include, but is not limited to the following.]

1. Assesses patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, family history, psychosocial situation, and cultural factors; and performing appropriate physical examination.

2. Orders/performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness.

3. Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks.

4. Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state laws. Handles minor lacerations.

5. Consults with physicians as needed and/or refers to physicians for more complicated medical cases or cases that are not a routine part of scope of work.

6. Monitors therapies and provides continuity of care.

7. Triages patient calls and evaluates patient problems. Responds to emergencies including use of CPR.

8. Instructs patient/family regarding medications and treatments. Educates patients regarding health promotion/illness prevention. Recommends appropriate community resources to meet patient/family needs.

9. Communicates appropriate case management information to other professionals and community agencies.

10. Prepares documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in written/computerized manner.

11. Participates in peer reviews, chart reviews, staff education, clinical guideline development, and other continuing education and quality assurance activities to demonstrate compliance with standards, regulations, policies, and procedures.

12. Acts as consultant/collaborator to clinical team on area of specialty. Complies with patient confidentiality requirements. Promotes patient advocacy.

13. Manages acute and long-term medical needs of patient. Provides monitoring and continuity of care between visits according to treatment plan including triaging patient calls/e-mails.

14. Collaborates with multidisciplinary team members by making appropriate referrals.

15. Understands and complies with OSHA and any other regulatory requirements.

16. Follows BSSP policies and procedural guidelines.

17. Performs other duties as assigned.

QUALIFICATIONS:

KNOWLEDGE OF:

1. Advanced practice of nursing care (nurse practitioners) or medical model and roles of physicians and physician assistants (physician assistants).

2. Medical terminology, anatomy and physiology, pathophysiology, pharmacology and therapeutics, basic nutrition, behavioral factors, psychosocial/family systems, diagnostic testing, interpretation of results and clinical decision making, alternative treatment modalities.

3. Knowledge of diagnosing and treating medical problems and developing care plans.

4. Patient assessment techniques including taking medical histories, performing physicals, evaluating health status including state of wellness, and compliance with care recommendations.

5. Medical equipment and instruments used to administer patient care.

6. Health promotion, health risk identification, and patient education principles and techniques.

7. Community, professional, and educational resources.

8. Practice management systems including electronic health record systems (EHRs).

9. Common safety hazards and precautions to establish/maintain a safe work environment.

SKILLS IN:

1. Oral and written communication; interviewing to gather medical histories; providing presentations, consultations, facilitation, and documentation, e.g., writing medical orders, patient education materials, and medical record updates.

2. Case management, time management, problem solving, crisis intervention, multitasking, and other organizational matters.

3. Modifying methods and techniques of care and in initiating appropriate emergency response and investigatory procedures.

4. Preparing and maintaining patient records.

5. Performing efficient and effective venipunctures and other laboratory procedures.

ABILITY TO:

2. Interpret, adapt, and apply guidelines and procedures.

3. Communicate clearly (orally and in writing) and establish/maintain effective working relationships with patients and clinical staff.

14. Competently use Microsoft Office, including Outlook, Word, PowerPoint, Excel, and appropriate practice management software.

15. Demonstrate eye–hand coordination, full range of motion, and manual dexterity.

Equipment Operated: Standard medical examination room instruments and equipment. Blood draw equipment including syringes, tubes, bandages, and other appropriate supplies. Notebook or desktop computer and peripheral equipment necessary to access computer/health information management system and Microsoft Office suite of products.

EXPERIENCE AND TRAINING:

1. Bachelor’s degree.

2. Mid-Level Providers as:

a. Physician Assistant:

i. Successful completion of accredited physician assistant program.

ii. National certification from the National Commission on Certification of Physician Assistants

iii. Active California Physician Assistant license

b. Nurse Practitioners:

i. Successful completion of accredited nurse practitioner program.

ii. Active California Registered Nurse license

3. Certified Medical Examiner

a. Federal Motor Carrier Safety Administration (FMCSA) medical examiner certification credential and active listing within the National Registry of Certified Medical Examiners within six months of the first date of an assignment of 50% or more of full time equivalency.

i. If not currently certified and actively registered, successful completion of training and passage of the National Registry Medical Examiner Certification Test is required within six (6) months of the first date of full-time employment. Forty (40) hours of leave will be provided for test preparation and completion.

4. Current CPR, BLS and First Aid certificates.

5. One year of experience as mid-level provider, preferably in clinic setting.

6. Possession of a valid California driver’s license, good driving record, and auto insurance.

PHYSICAL REQUIREMENTS:
