Job Description Summary:TrueNorth Search Firm has been retained to find top talent in wound care. We are seeking dedicated Wound Care Providers to join a dynamic healthcare team. You will be at the forefront of advanced wound care, G-tube management, and more, delivering exceptional care directly in patients’ residences. This is a mobile wound care role serving the home health, assisted living facilities and SNF sector of healthcare
Full training is provided to ensure your success in this vital role.
Essential Job Functions:
- Lead Multidisciplinary Teams: Conduct weekly rounds at healthcare facilities and patient homes.
- Provide Comprehensive Care: Deliver wound care services in residential homes, skilled nursing facilities, assisted living homes, and for home health and hospice patients.
- Assess Health Status: Review patients’ health and medical history.
- Diagnostic Studies: Order, interpret, and evaluate laboratory and diagnostic studies.
- Develop Treatment Plans: Create personalized wound care plans based on diagnoses.
- Perform Procedures: Execute bedside wound care procedures, including debridement, surgical cleansing, dressing application, and more.
- Monitor Progress: Track the healing progress of wounds.
- Maintain Productivity: Meet daily productivity expectations.
- Utilize Advanced Modalities: Manage various wound types using advanced techniques such as skin grafts, ultramist, and excisional debridements.
- Document Accurately: Keep precise and timely electronic medical records.
- Engage Nurses: Collaborate with partner facility nurses for wound care.
- Collaborate Effectively: Work closely with nursing, medical, and other healthcare team members to ensure quality patient care.
- Champion Customer Service: Excel in delivering outstanding customer service.
- Motivate and Encourage: Inspire and support patients and their caregivers.
- Provide Clear Direction: Foster a team environment that promotes commitment and engagement.
- Attend Team Meetings: Participate in regular meetings for ongoing training, team building, and leadership development.
