Physician Assistant or Nurse Practitioner – Internal Medicine (Skilled Nursing Facilities)
We are seeking a Physician Assistant or Nurse Practitioner with Internal Medicine and Skilled Nursing Facility (SNF) experience to join our established multi-specialty medical group. This role provides ongoing internal-medicine coverage for post-acute and long-term care patients across our partnered SNFs in the Arcadia and San Gabriel Valley areas, with flexibility for partial remote work.
As an NP or PA, you will manage a panel of medically complex SNF patients, perform comprehensive internal-medicine evaluations, proactively monitor changes of condition, and coordinate acute care plans throughout the course of an issue. The ideal candidate takes initiative, anticipates needs, communicates clearly with families, and works collaboratively with the IDT to deliver proactive, patient-centered care. This role includes occasional after-hours call (low acuity, phone-based).
We provide strong operational support including care coordination, billing/compliance support, and dedicated back-office staff.
Responsibilities
- Provide internal-medicine evaluation and management for SNF residents
- Conduct physical exams and ongoing chronic disease and acute change-of-condition management
- Order and interpret diagnostics; prescribe and manage medications
- Develop individualized, proactive care plans aligned with SNF regulations and patient needs
- Take initiative in monitoring and managing changes of condition throughout the acute episode
- Communicate confidently and clearly with patient families regarding updates and care plans
- Collaborate closely with the Interdisciplinary Team (IDT) including nursing, therapy, social services, and facility leadership
- Document in the EMR efficiently and meet regulatory/SNF requirements
- Participate in occasional after-hours call (phone-based)
Qualifications
- Valid and active Nurse Practitioner or Physician Assistant-Certified license
- Graduate degree from an accredited NP or PA program
- Active DEA license
- Required: SNF, geriatric, or post-acute care experience
- Strong internal medicine diagnostic and clinical management skills
- Exceptional communication skills, particularly with patient families and IDT teams
- Proactive problem-solving abilities and comfort managing change-of-condition workflows
- EMR proficiency and ability to work in a tech-based environment
- Ability to work independently within a team-based and partially remote structure
- Willingness to travel to 1–2 SNF facilities in the Arcadia/San Gabriel Valley region
- Chinese-speaking (Mandarin or Cantonese) is a plus due to patient population needs

Job Type: Full-time
Pay: $155,000.00 – $165,000.00 per year
