Physician Assistant or Nurse Practitioner – Primary Care / Endocrinology Location: 3737 Lone Tree Way, Suite B, Antioch, CA 94509 Training Location: 2182 East Street, Concord, CA 94520 Job Type: Full-time | Pay: $65.00 – $77.00 per hour Expected Hours: 40 per week | Schedule: Monday – Friday (No nights or weekends)

We are a growing, patient-focused multispecialty medical group dedicated to delivering high-quality care across Northern California. We are currently seeking a skilled and compassionate Physician Assistant (PA) to join our team in Antioch, with training provided at our Concord location.
This role focuses on primary care and endocrinology, especially the management of chronic conditions such as diabetes, hypertension, and lipid disorders.
Position Summary
As a Physician Assistant, you will play a key role in delivering comprehensive primary and preventive care. You will collaborate closely with physicians and specialists to manage patients with diabetes and other endocrine conditions while also supporting the general health needs of adult and geriatric patients.
Key Responsibilities

Conduct physical examinations and patient assessments

Prescribe medications and develop treatment plans

Monitor and manage chronic conditions, including diabetes, blood pressure, and cholesterol

Facilitate referrals and coordinate care across healthcare settings

Communicate with collaborating physicians and specialists regarding patient needs

Provide education and preventive care for diabetic and endocrine patients

Document all encounters in Epic EMR
Qualifications
Required:
-
Master of Science Degree from an accredited Physician Assistant program

Active California Physician Assistant license (must be in place before start date)

Strong communication, teamwork, and leadership skills

Ability to build rapport with patients and thrive in a fast-paced clinical environment
Preferred:
-
Experience in outpatient care

Background in adult and senior primary care

Familiarity with Epic EMR
