Harmony Health Medical Clinic and Family Resource Center (Harmony Health) has 4 locations serving the communities of Yuba and Sutter counties. Harmony Health offers full-service family practice medical care. Additionally, our Clinic on Wheels visits Camptonville and North San Juan weekly.

Position Summary: Harmony Health is seeking experienced Family Nurse Practitioners or Physician Assistants to deliver high-quality, patient-centered care under the supervision of a physician. Providers care for patients across the lifespan in a supportive, collaborative environment. This role will be responsible for seeing patients independently and collaborating with other providers, nurses, and medical assistants. After completion of training (up to 6 months), this role will see 16-20 patients/day (8-hour day) or 22-25 patients/day (10-hour day). Providers work closely with care teams to deliver effective and evidence-based interventions that support whole-person care.

Essential Responsibilities

· Perform history and physical exams on new and established patients, as well as patients being seen for follow-up appointments

· Provide follow-up and health maintenance care to patients in accordance with established protocols

· Initiate appropriate treatment and testing required for evaluation of illness based on training and experience

· Provide counseling and instruction to patients regarding health-related issues

· Document clinical care of the patient according to established guidelines

· Perform therapeutic and diagnostic procedures as indicated by training and experience

· Communicate with patients and family members within state law confidentiality and HIPAA guidelines concerning health-related questions

· Participate in continuous quality improvement process and other initiatives in the area of employee practice

· Other duties as assigned

Qualifications

· Valid California Medical License (Required)

· Valid DEA Certificate (or ability to obtain)

· Board Certified (Required)

· Valid BLS (Required)

· Up-to-date immunizations for Hep B, Tdap

· Overall knowledge of medical office patient care

· Knowledge of confidentiality issues involved in the release of medical information

· Must have excellent interpersonal skills, including the ability to interact with staff and providers at other medical facilities

Schedule & Work Environment:

· Outpatient clinical setting

· May include evening or weekend hours

· Productivity expectations consistent with FQHC standards

· Occasional travel between clinic locations may be required

Physical Requirements

· Ability to sit or stand for extended periods

· Occasional bending, reaching, and lifting up to 25 lbs

· Adequate vision and dexterity to complete documentation and clinical assessments

· Work performed in a standard clinical environment with moderate noise
