Mend & Restore is a mobile wound care practice serving patients across Greater Los Angeles. We bring advanced wound care directly to patients in their homes, skilled nursing facilities, assisted living communities, and senior care settings — eliminating the burden of transport while delivering clinic-quality care. Our team of wound-certified Nurse Practitioners and Physician Assistants operates under physician supervision, guided by a shared commitment to clinical excellence and patient dignity. THE OPPORTUNITY
We are seeking experienced, motivated NPs and PAs to join our growing PRN clinical team. You will carry an independent patient panel across our service area, performing comprehensive wound assessments and advanced wound care procedures in the field. This role is ideal for a clinician who values clinical autonomy, flexible scheduling, and the profound reward of meeting patients where they are — literally and figuratively.
WHAT YOU WILL DO
Perform comprehensive wound assessments and develop individualized wound care plans
Execute advanced wound care procedures including sharp and selective debridement, NPWT management, and skin substitute application
Manage complex wound types including diabetic foot ulcers, venous leg ulcers, pressure injuries, and post-surgical wounds
Order and interpret relevant diagnostic studies; coordinate with referring physicians and specialists
Document clinical encounters thoroughly in our EMR with accuracy and timeliness
Communicate proactively with patients, families, caregivers, and facility staff
Support Medicare Part B compliance and participate in quality improvement initiatives
Represent Mend & Restore with professionalism and warmth at every patient encounter

WHAT WE ARE LOOKING FOR
Active, unrestricted California NP or PA license in good standing
Current DEA registration and BLS/CPR certification
Minimum 2 years of advanced practice experience; wound care experience strongly preferred
Proficiency in wound assessment, debridement, and NPWT management
Familiarity with Medicare Part B billing and documentation requirements
Reliable personal transportation and willingness to travel across our service area
Strong autonomous clinical decision-making with a collaborative spirit
Exceptional communication skills — with patients, families, and care teams alike
A genuine belief that every patient deserves attentive, dignified care
PREFERRED QUALIFICATIONS
CWCN or WCC certification
SNF / ALF / home health experience
Diabetic foot ulcer management
NPWT experience
Personal injury lien-based care
