Guardian Home Health Care & Hospice, Inc. is seeking a compassionate and dedicated Medical Social Worker to join our home health team serving patients throughout San Joaquin County.
Responsibilities
- Conduct psychosocial assessments for home health patients
- Provide counseling, crisis intervention, and emotional support to patients and families
- Connect patients with community resources and assist with care planning
- Collaborate with the interdisciplinary care team
- Assist with advance directive planning
- Maintain timely and compliant documentation per Medicare guidelines
Qualifications
- Master’s degree in Social Work (MSW)
- Current CA licensure (ASW or LCSW)
- 1+ year experience in a healthcare setting
- Knowledge of Medicare home health regulations
- Valid CA driver’s license and reliable transportation
- Bilingual a plus
Pay: From $80.00 per hour
Work Location: On the road
