Occupational Therapist Assistant (COTA) – Travel Contract Location: San Jose, CA (Not the right fit? We have additional COTA opportunities available throughout California.) Pay: $1,800-1,900 per week Contract Length: 13 weeks (extension possible) Schedule: Day shifts | 36–40 hours per week
We’re Hiring! We’re looking for a motivated and compassionate Occupational Therapist Assistant (COTA) to join our Skilled Nursing Facility in San Jose, CA. Responsibilities

Provide COTA-assisted therapy interventions under supervision in skilled nursing settings.

Assess patients’ functional abilities and progress toward targeted outcomes.

Develop and implement individualized treatment plans aligned with goals.

Document treatment sessions, progress notes, and outcomes in medical records.

Collaborate with OT team, nursing staff, and physicians to coordinate care.

Utilize data and metrics to drive therapy effectiveness and target achievement.

Maintain compliance with facility policies, safety standards, and regulatory requirements.

Educate patients and families on therapy plans, goals, and home exercise programs.

Identify opportunities for professional development and pursue skill advancement.

Support business growth by contributing to patient pipelines and service expansion.

Participate in regular team meetings to review targets and performance against plans.

Demonstrate accountability for individual results within a collaborative team.
Experience/Qualifications

Licensed Occupational Therapy Assistant (COTA) in good standing.

Experience providing COTA-assisted therapy in skilled nursing or long-term care.

Ability to assess functional status and track progress toward goals.

Develop and implement individualized treatment plans with measurable targets.

Maintain compliance with facility policies, safety standards, and regulatory requirements.

Accurate documentation of sessions, progress notes, and outcomes in EMR.

Collaborate with OT team, nurses, and physicians to coordinate care.

Utilize data and metrics to drive therapy effectiveness and target achievement.

Educate patients and families on therapy plans and home exercise programs.

Engage in ongoing professional development and skill advancement.

Contribute to patient pipelines and service expansion to support growth goals.

Strong time management to manage individual targets within a collaborative team.
