We are seeking a dynamic and compassionate Wound Care Nurse Practitioner (NP/PA) to join our dedicated healthcare team. In this vital role, you will provide expert wound management and advanced nursing care to patients in the home setting. Your expertise will help improve healing outcomes, enhance patient comfort, and promote overall well-being. Duties
- Conduct comprehensive wound assessments, including evaluating wound size, depth, exudate, and signs of infection using advanced clinical techniques.
- Develop and implement personalized wound care plans utilizing evidence-based practices, including advanced dressings, debridement, and skin grafting when appropriate.
- Coordinate with multidisciplinary teams to ensure optimal patient outcomes, including collaborating with physicians, physical therapists, and case managers.
- Manage complex cases involving diabetic foot ulcers, pressure injuries, venous stasis ulcers, and other chronic wounds in various settings such as assisted living facilities and in-home settings.
- Educate patients and caregivers on wound prevention techniques, proper dressing changes, infection control measures, and lifestyle modifications to support healing
Skills
- Extensive knowledge of physiology related to skin integrity, tissue repair processes, and infection control protocols.
- Proficiency in advanced wound care techniques including debridement (autolytic and sharps).
- Strong background in managing complex wound cases involving hospice & palliative medicine considerations, and infectious disease care.
- Experience working in nursing homes or home care settings.
- Experience with collaborating with PCP’s, specialists and other care team members.
Pay: $140,000.00 – $155,000.00 per year

Experience:
- Wound Care as NP: 1 year (Required)
- Sharp Debridement: 1 year (Required)
- Wound care experience in a wound care clinic or hospital setting
- Home health experience (a plus)
- Experience with a Wound Vac or NPWT (a strong plus)
- Military experience (a plus)
License/Certification:
- Nurse Practitioner or Physician Assistant License (Required)
- Wound Care Certification, such as CWON, CWS, WCC, WOCNBC (a strong plus)
Ability to Commute:
- San Diego County (Required)
