We are seeking a dedicated Outpatient Physical Therapist Assistant to join our healthcare team on a consistent per diem basis serving a Memory Care Community.
This position offers flexibility and the opportunity to work with a collaborative care team, providing essential therapy services to a broad patient population. The ideal candidate will possess strong clinical skills, a compassionate approach, and experience working with various age groups and conditions, including geriatrics and memory care. This role is vital in helping patients regain independence and improve their quality of life through personalized therapy plans.
Duties
Assess patients’ mobility, functional abilities, and response to treatment interventions and implement individualized care plans developed by the supervising Physical Therapist aligned with patient goals and medical needsImplement therapeutic interventions in a senior living and in-home setting under the direction of a licensed Physical TherapistCollaborate with multidisciplinary teams including physical therapists, speech therapists, physicians, and caregiversManage case documentation accurately and maintain detailed patient records in compliance with healthcare regulationsEducate patients and families about treatment goals, progress, and strategies for continued improvement as directed by the supervising Physical TherapistParticipate in ongoing professional development activities to stay current with best practices in physical therapy assistance
Requirements
Valid Physical Therapist Assistant license in the state of CaliforniaExperience working in outpatient clinics; additional experience in inpatient, skilled nursing facilities, home health, or school-based settings is preferredKnowledge of engaging patients with cognitive deficitsFamiliarity with working with diverse populations including geriatrics, individuals with autism or disabilitiesStrong understanding of physiology, anatomy, medical terminology, and patient care principlesExcellent communication skills for collaborating with healthcare teams and educating patients and familiesCompassionate attitude with a commitment to improving patient outcomes and quality of life
