Job Summary
We are a growing rheumatology practice focused on autoimmune disease management and patient-centered chronic care. We are seeking a part-time bilingual (Spanish-speaking) NP or PA to support follow-up care and medication management for our autoimmune patient population.
Responsibilities:
- Follow-up visits for stable rheumatology patients
- Medication management and monitoring (DMARDs, biologics)
- Patient education in English and Spanish
- Coordination with supervising physician
- Lab review and protocol-based adjustments
Requirements:
- NP or PA license in California (or eligible)
- Fluent Spanish required
- Outpatient experience preferred
- Strong communication and chronic care management skills
Schedule: 1-2 days/week (flexible)
Responsibilities
- Conduct thorough patient evaluations, including vital signs, physical examinations, and medical history assessments tailored to rheumatology patients.
- Administer injections such as corticosteroids or biologics and perform procedures like joint injections or aspiration under sterile technique.
- Manage medication plans, including prescribing and monitoring treatments such as DMARDs (Disease-Modifying Anti-Rheumatic Drugs), and educate patients on their use.
- Coordinate care with multidisciplinary teams, including specialists in internal medicine, orthopedics, and physical therapy to optimize patient outcomes.
- Document all patient interactions accurately using EMR (Electronic Medical Record) systems like Epic or eClinicalWorks while ensuring compliance with HIPAA regulations.
- Assist in clinical research projects related to rheumatologic therapies and contribute to data collection and analysis efforts.
- Provide patient education on disease management strategies, lifestyle modifications, and medication adherence in both English and Spanish.
Experience
- Proven experience working as a Nurse Practitioner or Physician Assistant in outpatient clinics or hospital settings with a focus on rheumatology preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth for documentation and order entry.
- Strong background in performing clinical procedures including suturing, IV infusion, phlebotomy, and specimen collection/processing.
- Experience with telehealth platforms to deliver remote consultations effectively.
- Knowledge of pharmacology related to autoimmune diseases and familiarity with CPT coding for billing purposes.
- Prior exposure to working with diverse populations including seniors in senior care or assisted living environments is advantageous.
