Job SummaryWe are seeking a dedicated and highly skilled Nurse Practitioner with specialized experience in podiatry services to provide comprehensive foot and lower extremity care in nursing homes and assisted living facilities across Northern California, including the Bay Area, South Bay, Sacramento, the Central Valley, and Chico.
The ideal candidate demonstrates strong clinical expertise in assessing, diagnosing, and managing a wide range of podiatric conditions, including diabetic foot care, wound management, nail and skin disorders, and preventative care. This role focuses on delivering compassionate, patient-centered care that improves mobility, comfort, and overall quality of life—particularly for elderly patients and those with limited access to traditional clinic settings.
Key responsibilities include performing routine and advanced podiatric procedures, developing individualized care plans, and collaborating effectively with interdisciplinary healthcare teams. Success in this role requires excellent communication and organizational skills, as well as the ability to work independently in a mobile care environment with strong time management and adaptability.
The candidate must be committed to maintaining high standards of clinical excellence, infection control, and patient education while ensuring compliance with healthcare regulations and best practices.
CompensationCompensation is flexible and can be structured as:
- Per-patient rate plus productivity bonus
- Percentage of collections
- Base salary starting at $110,000 plus bonus

Duties
- Perform comprehensive assessments of patients’ feet and lower extremities in home, assisted living, and community settings
- Manage common podiatric conditions, including diabetic foot complications, ulcers, infections, calluses, and nail disorders
- Provide routine foot care services such as nail trimming, debridement, and treatment of skin conditions
- Conduct wound care management, including cleaning, dressing, and monitoring healing progress
- Develop and implement individualized treatment and care plans based on patient needs and medical history
- Prescribe medications and treatments in accordance with state regulations and clinical guidelines
- Educate patients, caregivers, and families on foot hygiene, preventative care, and disease management
- Monitor and manage chronic conditions affecting foot health, particularly diabetes and vascular issues
- Maintain accurate and timely clinical documentation using electronic health records
- Travel to patient locations while ensuring proper setup and sanitation of mobile equipment
- Collaborate with physicians, podiatrists, nurses, and other healthcare professionals to coordinate care
- Identify high-risk patients and initiate appropriate referrals when necessary
- Ensure compliance with infection control standards, safety protocols, and healthcare regulations
- Manage scheduling, supply inventory, and equipment needed for mobile practice
- Provide compassionate, patient-centered care that supports mobility, independence, and overall well-being
- Refer patients to vascular, imaging and home health
Skills
- Proven experience working with electronic health record (EHR) systems such as Practice Fusion mobile documentation systems
- Advanced clinical assessment and diagnostic skills for foot and lower extremity conditions
- Able to see multiple patients at one location, averaging about 20-50 patients a day
- Proficiency in podiatric procedures, including nail care, debridement, and wound management
- Strong knowledge of diabetic foot care and prevention strategies
- Ability to develop and implement individualized treatment plans
- Excellent patient education
- Strong critical thinking and clinical decision-making abilities
- Effective communication and interpersonal skills with patients, families, and healthcare teams
- Ability to work independently in mobile and non-traditional care environments

Pay: $110,000.00 – $140,000.00 per year
