Overview
Medi Mobile Group is looking for a compassionate and reliable Nurse Practitioner or Physician Assistant to join our healthcare team. In this role, you will deliver high-quality, patient-centered care across a variety of settings, including patients’ homes, helping ensure care is both accessible and convenient. The ideal candidate is flexible, comfortable with travel, and committed to providing personalized care in home environments.

Responsibilities include diagnosing and treating patients, developing individualized care plans, and collaborating with other healthcare professionals to support the best possible outcomes.

Duties

  • Perform comprehensive in-home assessments and physical exams, adapting care to each patient’s living environment.
  • Evaluate, diagnose, and manage both acute and chronic conditions with a focus on accessible, convenient care delivery.
  • Design and carry out personalized treatment plans that reflect individual patient needs within home settings.
  • Educate and support patients and their families with clear, practical health guidance and coaching.
  • Work collaboratively with physicians, nurses, and interdisciplinary team members to ensure coordinated care.
  • Carry out routine in-home procedures and coordinate appropriate follow-up services.
  • Maintain thorough, accurate documentation in compliance with healthcare regulations and standards.
  • Travel to patient homes and community locations as required, effectively managing schedules and time.

Qualifications

  • Valid Nurse Practitioner or Physician Assistant license with current certification; experience in primary care, internal medicine, family practice, or urgent care preferred.
  • Demonstrated proficiency with EMR/EHR systems.
  • Experience working with diverse populations.
  • Strong skills in medical documentation.
  • Excellent communication skills for patient education on health coaching topics like nutrition or disease management; experience working with individuals with disabilities or developmental challenges is a plus.
  • Valid driver’s license and active insurance required.
  • Has to dedicate minimum one weekend a month.
  • Multi-lingual preferred
  • NPI + Pecos Enrollment
  • BLS / ALS Certification
  • DEA Registration
