Fallbrook Medical Center is a trusted local clinic providing adult primary care for ages 18+ in Fallbrook and surrounding communities. We offer bilingual, insurance-friendly care focused on routine visits, same-day care for minor conditions, chronic condition management, and support for seniors and Medicare patients.
We are seeking a bilingual Nurse Practitioner or Physician Assistant to join our team full-time. The ideal candidate is confident in outpatient primary care, comfortable working independently, and committed to delivering clear, compassionate care to a diverse community.
Responsibilities
- Provide primary care services for adult patients 18+
- Evaluate and treat routine medical concerns and minor same-day conditions
- Perform patient histories, physical exams, assessments, and diagnoses
- Manage chronic conditions and develop treatment plans
- Prescribe and manage medications as appropriate
- Educate patients on treatment plans, prevention, and follow-up care
- Coordinate referrals to specialists and community resources
- Maintain accurate, timely, and compliant medical documentation
- Collaborate with supervising physician(s) as required
- Participate in quality improvement initiatives
Qualifications
- Minimum 2 years of full-time experience as an NP or PA in an outpatient primary care setting
- Fluent in English and Spanish, oral and written
- Comfortable working independently
- Graduate of an accredited NP/RN or PA program
- Master’s degree in Nursing required for NPs
- Active California NP or PA license
- NPI
- BLS certification
- Current DEA registration
Preferred
- Experience with multicultural and Spanish-speaking patient populations
- Experience in adult primary care and chronic condition management
- Familiarity with senior care and Medicare patients
- Interest in integrative or functional medicine is a plus
Schedule and Work Setting
- Full-time
- 8-hour shifts
